I can't fill in your paper, as I've been out of the industry too long, but one of the first things we were taught, was to do a risk assessment of our working place before starting work. Check the roof, check the sides, if there is a problem, if it's within the scope of our ability, to correct the problem, if not report it straight away to a mine official, or other senior mining offical.

Cheers, Oomay Just a thought, the Lithgow State Mining Museum, has a page on Facebook, you could try there. Lithgow still has three working collieries, and I'm sure there must be some who visit the page who are still working in the industry.
